you will receive a new policy number
and a membership card.
To ensure you receive your new membership card, please update your postal address by calling us
If you visit a medical or health clinic you will need to provide them with your first name, last name, and date of birth so they can locate your new policy number. Otherwise, you will need to use your new membership card or policy number to claim your OSHC benefits.
Get your parents or guardian to complete the ‘Claim’ form located on the International Health
website and email it through with the relevant invoice or receipts to email@example.com
Your current membership card will be valid until the 15th of November 2020. From 16th November 2020, you will need to use your new policy number and e-membership card located on the Allianz MyHealth portal or mobile application.
